You will require a MyCity account to begin the application. If you don't have an account please Register For MyCity. This MyCity account will help you track the progress on your records request.

Payment by credit card will be required to complete the application process. If there are additional pages you may have to make an additional payment. If additional payment is required an email will be sent to your contact email with instructions.

Once the request is complete you will be emailed with any records we found. Alternatively you can request to come pick up your records at the 5th floor of City Hall.
